This agreement gives details of our key terms and conditions of the service we provide. The purpose of this is to ensure clarity and reduce any ambiguity. If you have any questions or need any clarification please do not hesitate to speak to a member of our team. Your access to and use of the Service is conditioned on your acceptance of and compliance with these Terms. These Terms apply to all visitors, users and others who access or use the Service. By accessing or using the Service you agree to be bound by these Terms. If you disagree with any part of the terms then you may not access the Service.
Our Team
All of our clinical team members (excluding trainee dental nurses) are registered with the GDC (General Dental Council) and are therefore compliant with regular training and updates following the GDC’s continued professional development recommendations.
Personal Details and Medical History
We ask all of our patients to complete a medical history form. This is very important as some medication and/or health conditions may affect your dental health. Should there be any changes in any way it is very important for you to tell your clinician. It is your responsibility to inform the clinician of changes in either your health, medication or personal details such as address or telephone contact numbers.
If you have any disability you think we need to be aware of please contact the practice before your appointments and we will try to make suitable arrangements to meet your needs.
All your personal details are stored securely in accordance with the GDPR Data Protection Regulations. No clinical or personal details will be released to a third party without your written consent. All clinical notes, radiographs, and photographs remain the property of The Eledent Clinic.
You may be contacted by the practice via the phone, text, e-mail or letter with regards to routine recalls, appointment reminders, special offers or changes at the practice. If you do not wish to be contacted by the practice by any of these means please let us know. You will have the opportunity to choose your preferences when completing the initial registration form.

Registration
Initial registration is completed following attending for a new patient dental check-up. If you have not attended for a check-up for over 24 months, your registration with the Eledent Clinic will lapse and you will need to re-register by attending for a new patient dental check-up as the conditions of the teeth and gums changes drastically over time.
The Eledent Clinic is a fully private dental services and all treatments are provided on a private basis only.
Consultations
All cosmetic or orthodontic consultations are free of charge if they are carried out virtually. However, a holding deposit will be taken to secure a booking time for the consultation if you want to attend for an appointment at the practice, which will be discounted from any subsequent treatment undertaken.
Treatment Planning
Once your treatment plan has been agreed with the Dentist, we will provide printed details of your plan. If this plan changes due to radiographic or clinical findings, we will inform you and discuss this with you. Treatment plans are valid for 90 days from the date the treatment was prescribed. Should it exceed this period, a new assessment may be necessary and there may be a change in the treatment and/or the fees. If this occurs, we will inform you and discuss this with you.
If there are any points on your treatment plan that you wish to query, please do not hesitate to ask us, before the appointments for actual treatments are made.
Fees
At the Eledent Clinic we require fees to be settled prior to or at the appointment where treatment is provided. Where treatment incurs a laboratory fee, at least 50% of the total fee is due at the appointment where impressions or digital scans are taken. Fees for certain treatments like Dental Implants and Invisalign® are taken in staged payments at each visit. We will discuss and plan a schedule of payment for you.
In cases where treatment is paid by direct debit, please be aware that the payment plan agreement must be accepted and signed prior to the start of the treatment. Please note that payment plans are only available on treatments over £1000, not including Emergency Dental Treatment, and that a minimum deposit of 30% is also be required. The balance of treatment fees must be completed in 10 months. In the event of any late payments more than one month, the total balance will be due and should be paid by the end of the month to avoid legal proceedings.
The fees displayed on any pages of our website are for guidance purposes and we reserve the rights to change them at any time. The cost of the treatment may vary depending of the complexity of the case and suitability of the patient. The patient will be provided with a full written treatment plan on their consultation with the dentist.
Payment methods accepted include cash, debit or credit card. Unfortunately, we do not accept any payments by cheque.
Fees for treatment where intravenous sedation is provided must be settled prior to the appointment to avoid financial transactions, or signatures being required, whilst a client is still under the influence of the sedative.
The Eledent Clinic reserves the right to charge time-based deposits for booking future appointments. Deposits are deductible off the cost of treatment.
Where a refund is to be processed for a card payment, it can only be done so using the original card that the payment was made from.
The Eledent Clinic reserves the right to make a charge for any debt passed to a debt collection agency.
All the fees applied for any form of Dental treatment at the clinic are based on the materials used, laboratory costs incurred (if applicable), and the clinical time spent.

Emergency Appointments
Should you need to see a dentist urgently, please contact the reception team as early as possible. Our phones are answered during our opening hours and we will respond to a message left on our answer service as soon as possible. Please ensure that your contact details are clear. Consent Forms
Certain treatments require completion of a written consent form. This is in order to explain the treatment, aftercare and any risk to you thoroughly, before any of these treatments are carried out.
Guarantee
At the Eledent Clinic, our dental fillings, crowns, veneers, implant crowns and dentures are guaranteed for a period of twelve months following placement, provided that the following conditions are met:
- The patient has fully paid for the treatment and does not owe the practice any money for the treatment received;
- The patient attends at the recommended intervals for a dental check-up and hygiene appointment (scale and polish), for example, every 6 months;
- The dental fillings, crowns, veneers, implant crowns and dentures have not been damaged as a result of an accident, trauma or excessive grinding;
- The patient has followed all post-treatment maintenance recommendations made by our dentists, including proper oral hygiene and diet advice. If the patient is diagnosed with tooth-grinding (bruxism) and a night guard is advised but not used daily, the guarantee is void;
- No treatment is guaranteed for more than 1 year, unless at the discretion of the dentist.
Some treatments may have a guarantee of less than 1 year or have no guarantees due to the nature of the treatment, such as scaling (hygiene treatment), and in such cases you will be informed by your Dentist either verbally or in writing, or both.
Use of Images and X-rays
The Eledent Clinic may use images and x-rays of your smile and teeth only, for educational purposes and/or marketing on the Eledent Clinic website, social media accounts, and on promotional and/or educational literature. Your name will never be published, and your identity will never be disclosed. However, if you DO NOT wish for us to use your images and x-rays in this way, please let us know.
Complaint’s Policy
At The Eledent Clinic we always take complaints about any aspects of our services very seriously, in order to ensure that every patient has only the very best experience at all times.
Complaints can be made by telephone or in writing via email or through our online website contact form, by the patient or by an authorised person on the patient’s behalf. Complaints should be made to the ‘The Eledent Clinic Management’, and should be clear, so that they can be dealt with efficiently.
Every complaint will receive a written/email acknowledgement within 2 days, and the Management will strive to resolve the complaint within a quick, reasonable period of time (usually about 2 weeks).
For our full complaints procedure and policy, please ask a member of our reception team. If a complaint is about any aspect of clinical care or associated charges it will normally be acknowledged by the Management team and then referred to the treating Dentist. Please note that in these cases, The Eledent Clinic accepts no liability on behalf of the treating Dentist and acts as a liaison between the patient and the treating Dentist, only.
No Tolerance of Abuse Policy
At The Eledent Clinic we operate a zero tolerance policy to abuse to our Dentists and staff, loud/disorderly/drunken behaviour, persistent missing and late cancellation of appointments (after multiple warnings). In these situations, The Eledent Clinic reserves the right to refuse treatment and admission.
GDPR Data Protection Regulation
We store all patient personal details on a secure computer system in accordance with The General Data Protection Regulation. All clinical notes, digital radiographs, digital photographs etc. remain the property of The Eledent Clinic. Copies of notes, radiographs and photographs can be made available on request, and The Eledent Clinic reserves the right to charge an administration fee for these, in the cases of excessive, unfounded, particularly if it is repetitive requests for data to be disclosed, in accordance with GDPR (May 2018).
Intellectual Property
The Service and its original content, features and functionality are and will remain the exclusive property of Eledent – Dentist in Sutton and its licensors.
Links To Other Web Sites
Our Service may contain links to third-party web sites or services that are not owned or controlled by Eledent – Dentist in Sutton.
Eledent – Dentist in Sutton has no control over, and assumes no responsibility for, the content, privacy policies, or practices of any third party web sites or services. You further acknowledge and agree that Eledent – Dentist in Sutton shall not be responsible or liable, directly or indirectly, for any damage or loss caused or alleged to be caused by or in connection with use of or reliance on any such content, goods or services available on or through any such web sites or services.

Termination
We may terminate or suspend access to our Service immediately, without prior notice or liability, for any reason whatsoever, including without limitation if you breach the Terms.
All provisions of the Terms which by their nature should survive termination shall survive termination, including, without limitation, ownership provisions, warranty disclaimers, indemnity and limitations of liability.
Disclaimer
Your use of the Service is at your sole risk. The Service is provided on an “AS IS” and “AS AVAILABLE” basis. The Service is provided without warranties of any kind, whether express or implied, including, but not limited to, implied warranties of merchantability, fitness for a particular purpose, non-infringement or course of performance.
Governing Law
These Terms shall be governed and construed in accordance with the laws of United Kingdom without regard to its conflict of law provisions.
Our failure to enforce any right or provision of these Terms will not be considered a waiver of those rights. If any provision of these Terms is held to be invalid or unenforceable by a court, the remaining provisions of these Terms will remain in effect. These Terms constitute the entire agreement between us regarding our Service, and supersede and replace any prior agreements we might have between us regarding the Service.
Changes
We reserve the right, at our sole discretion, to modify or replace these Terms at any time. If a revision is material we will try to provide at least 30 days notice prior to any new terms taking effect. What constitutes a material change will be determined at our sole discretion.
By continuing to access or use our Service after those revisions become effective, you agree to be bound by the revised terms. If you do not agree to the new terms, please stop using the Service.
